It has embarked on the \"virage ambulatoire\" (ambulatory shift): the transition from a healthcare system in which the hospital is at the heart of all care, to an organization that makes local doctors and primary care teams the coordinators of healthcare pathways. This has led to the development of a range of care modalities, such as outpatient surgery, day hospitalization (HDJ) and home hospitalization (HAD). The development of ambulatory care also involves a\u00a0<strong>less use of conventional hospitalization<\/strong>\u00a0in favor of other ways of caring for patients, whether in the community or at home. At a time when 50 % of hospital carers may have been affected by burnout, the challenge here is indeed to\u00a0<strong>restore the hospital to its rightful place and build healthcare pathways that mobilize the right player, for the right care, at the right time.<\/strong><\/p><\/div><\/div><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\"><p><strong>Ambulatory and domiciliary practices thus represent all of the devices that make it possible to limit the time spent in a healthcare facility.<\/strong>\u00a0This has been made possible in particular by the emergence of numerous\u00a0<strong>innovations<\/strong>\u00a0which have contributed to significant changes in healthcare pathways:<\/p><\/div><\/div><div class=\"standard-arrow list-divider bullet-top\"><ul><li style=\"margin-bottom: 0.5em;\"><strong>technical innovations<\/strong>Surgical robots, for example, improve surgical precision and healing, enabling patients to return home more quickly;<\/li><li style=\"margin-bottom: 0.5em;\"><strong>technological innovations<\/strong>These include remote monitoring devices, which allow more regular remote monitoring of chronic illnesses and, in some cases, avoid full hospitalization;<\/li><li style=\"margin-bottom: 0.5em;\"><strong>therapeutic innovations<\/strong>These include treatments and drugs that are less burdensome for patients and can be delivered in the community.<\/li><\/ul><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4521da1 elementor-widget elementor-widget-text-editor\" data-id=\"4521da1\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h4>Obstacles to the deployment of ambulatory and domiciliary practices<\/h4><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\"><hr style=\"border: 0; height: 2px; background-color: #000000; margin: 0 0 2em 0;\" \/><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\">Despite the development of new care methods,<strong>\u00a0France is still struggling to make the transition from ambulatory to residential care.<\/strong>\u00a0In fact, the average length of hospital stay (LOS) in France is higher than the OECD average.<strong>\u00a0Several obstacles<\/strong>\u00a0to the deployment of ambulatory and domiciliary practices can be highlighted in our country.<\/div><\/div><\/div><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-20f3858 elementor-widget elementor-widget-text-editor\" data-id=\"20f3858\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h5>Lack of coordination between players<\/h5><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\"><hr style=\"width: 70px; height: 2px; background: #00ade9; border: 0; margin: 0 0 2em 0;\" \/><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\">The new healthcare pathways require greater coordination between hospital, city and homecare professionals.\u00a0<strong>Geographical separation<\/strong>\u00a0of these professionals and\u00a0<strong>their multiplicity<\/strong>\u00a0are making it more complex to set up teamwork around the patient. The development of<strong>digital tools<\/strong>to help healthcare professionals get in touch with each other.<strong>emergence of coordinated exercises<\/strong>\u00a0in the city, represent\u00a0<strong>levers to be perpetuated to facilitate this coordination and build seamless care paths.<\/strong><\/div><\/div><\/div><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cf050d5 elementor-widget elementor-widget-text-editor\" data-id=\"cf050d5\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h5>Training insufficiently geared towards outpatient care<\/h5><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\"><hr style=\"width: 70px; height: 2px; background: #00ade9; border: 0; margin: 0 0 2em 0;\" \/><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\">The training of healthcare professionals (medical and paramedical) is poorly adapted to the issues raised by ambulatory and home care. Patients and their families\u00a0<strong>are not always aware of the various possibilities<\/strong>\u00a0treatment and care.<\/div><\/div><\/div><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-bf7056d elementor-widget elementor-widget-text-editor\" data-id=\"bf7056d\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h5>Financing and pricing models with few incentives<\/h5><div class=\"wpb_text_column wpb_content_element\"><div class=\"wpb_wrapper\"><hr style=\"width: 70px;
